1. INTRODUCTION
This Terms & Conditions page is for parents who are enrolling their children for Atlanta Slavic Kids Summer Camp. Please understand, that while registering attendees for Atlanta Slavic Kids Summer Camp you are agreeing to these Term & Conditions on a enrollment form. Please understand, disagreeing with any of the bellow Terms & Conditions sections, Atlanta Slavic Church / First Pentecostal Slavic Church, Inc. has a right to decline acceptance of your child(ren) to participate in Atlanta Slavic Kids Summer Camp.
2. TERMS & CONDITIONS
PRECAUTION
Please understand that in order for your child(red) to attend Atlanta Slavic Kids Summer Camp, you must agree to keep your child(ren) at home if they exhibit any sign of illness.
CONSENT TO TRIP
Please understand that you have to bring your child(ren) to a camp site on WEDNESDAY, JULY 19, 2023 between hours of 4:00 AM to 6:00 PM EST and pick up your child(ren) on SATURDAY, JULY 22 by 6:00 PM EST. This will require a physical trip using your own vehicle.
ALLERGIES AND MEDICAL CONDITIONS
If your child or any of your children have allergies or medical conditions you should disclose them in the registration form. For example: Peanut butter allergy or allergic to bee sting, or Asthma – needs inhaler.
MEDICAL AUTHORIZATION OF MINOR
You, as parent(s), certify that you have legal custody of the aforementioned minor child(ren) in the registration form. You grant your authorization and consent for Atlanta Slavic Church / First Pentecostal Slavic Church, Inc. volunteers, (hereafter “Supervising Adult”) to administer general first aid treatment for any minor injuries or illnesses experienced by the minor. If the injury or illness is life threatening or in need of emergency treatment, I authorize the Supervising Adult to summon any and all professional emergency personnel to attend, transport, and treat the participant and to issue consent for any X-ray, anesthetic, blood transfusion, medication, or other medical diagnosis, treatment, or hospital care deemed advisable by, and to be rendered under the general supervision of, any licensed physician, surgeon, dentist, hospital, or other medical professional or institution duly licensed to practice in the state in which such treatment is to occur. It is understood that this authorization is given in advance of any such medical treatment, but is given to provide authority and power on the part of the Supervising Adult in the exercise of his or her best judgment upon the advice of any such medical or emergency personnel. I verify that I and my child(ren) named above and on the sibling form are in good health and capable of participating in strenuous activities, and whenever necessary, will tailor my/their activities to those within the bounds of my/their physical health. This authorization is effective commencing on the 19 day of July, 2023 and expiring on the 22 day of JULY 2023.
INDEMNITY AGREEMENT
You, the parent and/or legal guardian of the child(ten) aforementioned int he registrations form and the undersigned hereby indemnifies and holds harmless Atlanta Slavic Church / First Pentecostal Slavic Church, Inc. volunteers from and against any and all claims, demands, actions, suits, losses, costs, charges, expenses, damages and liabilities whatsoever which Atlanta Slavic Church / First Pentecostal Slavic Church, Inc. may pay, sustain, suffer or incur by reason of or in connection with church camp, including, without limiting the generality of the foregoing, all costs and expenses (including legal expenses) incurred in connection with any such loss or damage.

Ticksy provides the support ticketing platform we use to handle support requests. 7. HOW LONG WE RETAIN YOUR DATA
When you submit a support ticket or a comment, its metadata is retained until (if) you tell us to remove it. We use this data so that we can recognize you and approve your comments automatically instead of holding them for moderation.
If you register on our website, we also store the personal information you provide in your user profile. You can see, edit, or delete your personal information at any time (except changing your username). Website administrators can also see and edit that information.
8. SECURITY MEASURES
